**I give permission for the above-named child to participate in a trial class at MDA House of Dance/Miss Darcy’s Academy. I understand this is a physical activity and will not hold MDA/Miss Darcy’s Academy LLC or instructors liable for any injuries or incidents during participation or while on or around the premises. I also grant permission for photos or videos taken during the class to be used for promotional purposes.*
